2. Download the Application and Pledge Form for the program you are interested in:
* Preschool Application and Pledge Form
* Toddler Program Application and Pledge Form
3. In Adobe Acrobat under “Tools,” select “Fill & Sign”.
4. Fill out the form, save it, and send it back to us at [email protected]. You may also mail it to the address on the form.
